In this personal training session, we focus on technique, balance, and agility.
Cancellation Policy
1. Membership Terms: This agreement reflects a pricing structure based on a 12-month membership. If a student plans to take time off, Diaz Isshinryu Combat Sports Academy may adjust the monthly fees according to the active participation period upon written approval. Students agree to pay for all scheduled classes regardless of attendance. The membership automatically renews unless written cancellation notice is provided 30 days **before the 1st of the month**. If no such notice is given, the member is responsible for the following month’s tuition. Payments late by more than 5 days from the due date will incur a $25 late fee. Returned checks will incur a $30 NSF fee. 2. Cancellation and Hold Policy: Members relocating more than 25 miles, experiencing long-term disability (with physician certification), or in the event of death may request cancellation. Temporary disability allows members to pause or extend their contract at no additional cost. All cancellation notices must be submitted in writing and delivered to 280 Merrimack St. Suite 157, Lawrence, MA 01843, or sent via certified mail within 3 business days of signing. 3. Attendance & Curriculum: Diaz Isshinryu Combat Sports Academy reserves the right to modify class schedules, curriculum, and student capacity. Promotion or rank is not guaranteed within any specific time and is based on progress and instructor evaluation. 4. Protective Gear Policy: All students must purchase sparring gear directly from the school. Outside equipment is not allowed due to sponsorship agreements and school safety policies. 5. Liability Waiver: By signing, I release Diaz Isshinryu Combat Sports Academy, its instructors, staff, and affiliates from any liability related to injury, damage, or loss during martial arts, exercise, or related activities. This release applies even in cases of negligence and covers all programs: Karate, Muay Thai, Cardio Kickboxing, Kobudo, Boot Camp, and Kids Night Out. 6. Media & Communication Consent: I authorize the use of any photos or video taken during classes or events for promotional purposes. Diaz Isshinryu Combat Sports Academy may contact me via phone, text, or email regarding updates, events, or programs.
